APPOINTMENTS AND CANCELLATIONSPatients may call our office during business hours to schedule an appointment. New patients are welcome and encouraged to join our practice. We strive to value your time and attempt to be as prompt as possible. Since appointed times are reserved exclusively for each patient we ask that you please notify our office as soon as possible if unable to keep your appointment, ideally at least 48 hours in advance. Another patient, who needs our care, could be scheduled if we have sufficient time to notify them. We realize that unexpected things can happen, but we ask for your assistance in this regard.
Your initial appointment will consist of a consultation explaining your diagnosis and treatment options. Occasionally, treatment can be performed the same day as the consultation. However, a complex medical history or treatment plan may require an evaluation and a second appointment to provide treatment on a separate day. Please assist us by providing the following information at the time of your consultation: * X-rays (if applicable) * A list of your current medications * If you have medical or dental insurance, bring the necessary completed forms. This will save time and allow us to help you process any claims. Please alert the office if you have a medical condition that may be of concern prior to surgery (i.e,. diabetes, high blood pressure, artificial heart valves and joints, rheumatic fever, etc.) or if you are on any medication (i.e., heart medications, aspirin, anticoagulant therapy, etc.) . X-Rays If you have x-rays from a previous dentist or physician, you may request that they forward them to our office. If there is not enough time, please pick them up and bring them to our office. If additional films are necessary, they can be taken at our facility. ** IMPORTANT: A parent or guardian must accompany all patients under the age of 18 at the consultation visit**
We feel it is important to provide financial estimates for the cost of proposed treatment. For those who have dental insurance, we do accept assignment of benefits for most plans. Payment is expected at the time of treatment, including deductibles and co-payments. For your convenience, we accept payments of cash, check, or credit card (Master Card, Visa, Discover, and American Express) as well as extended payment plan options through Care Credit. Depending upon the recommended treatment, there can be a substantial investment for the proposed care, including orthodontics, and we are prepared to assist you in arranging for a plan that suits your needs.
If we are providers for your insurance plan, we will bill your insurance directly and collect your portion at the time of service. If we are not a party to the contractual arrangement with your insurance company, we do want to help you receive the maximum reimbursement to which you are entitled. As a convenience to you, we will help you process your insurance claims in order for you to receive the maximum benefit. We will also gladly provide dental x-rays and a written diagnostic report should your insurance company have any questions about the services provided.
